12/28/2018 The Append Progress Notes form has been disabled. Instead, if you want to add an additional information to a finalized progress note, use the form Append Documents.
11/6/2018 CSI questions on the second tab of the Avatar Diagnosis form must be answered when adding a diagnosis to the chart.
9/14/2018 As described previously, non-LPHA plan authors will continue to submit the SC MH Episodic Treatment Plan in draft (no finalizing). LPHA will then open plan, finalize and submit. A few changes have been made to the plan to facilitate this process.
8/9/2018 Non-LPHA authors will submit the SC MH Episodic Treatment Plan in draft. LPHA will open plan, finalize and submit. This helps make the LPHA signature date clear and easy to find. The LPHA signature date is the start date of the plan.
