Adolescent
Family Life Program (AFLP)
Public Health Nurses provide home visits to
pregnant and parenting teenage women under age 20, teenage fathers
up to age 21, and their children. Services include assessments,
education, and case management for teens to promote a healthy
pregnancy and baby, to stay in school or re-enroll in school, to
postpone future pregnancies, and to become emotionally, socially,
and financially independent.

Child
Passenger Safety Program
This program
provides community education and information about child passenger
safety, including, proper use and installation of child
safety seats, information about current laws and recommendations
for safety practices, trainings and technical assistance
for health and child care professionals and public
service agencies, and car seats for low-income families,
as funding is available.

Comprehensive
Perinatal Services Program (CPSP)
Health
care practitioners in the community provide prenatal
care that also includes assessments, education, childbirth
education classes, support and referrals for other needed
services. All pregnant Central Coast Alliance for
Health members and pregnancy-only Medi-Cal recipients are eligible to
receive CPSP services.

High
Risk Infant Program (HRI)
Public Health Nurses provide home-based support, education,
and professional assistance for families with premature and/or
substance exposed babies, or moms with mental health issues. The
program is aimed at providing for the infant’s optimal cognitive,
emotional, and physical development, and ensuring comfort and
competency of family members in parenting and care-taking roles.

Together Program (FT)
A new program designed in collaboration with Health
Services Agency, Human Resources Agency, Santa Cruz Community
Counseling Center and the Family Resource Centers to provide
families with services and support to prevent children from entering
the Child Protective Services.

Primeros
Pasos Program (PP)
A
new program designed in collaboration with Health Services Agency,
Human Resources Agency, Santa Cruz Community Counseling Center and the
JANUS Recovery Program to provide comprehensive assessment, intensive
case management and treatment opportunities to families where parental
substance abuse is a significant concern.

Pregnancy
Outreach and Education (POE)
Program
provides education, information, referrals and coordination to
assist pregnant women in obtaining early
and comprehensive prenatal health care and other needed services. In
particular, program assists pregnant women with substance use
and/or mental health concerns.

Infant Death Syndrome Program (SIDS)
Public Health Nurses provide support, referrals and coordination of
services for families, child care providers and others who have experienced the
loss of a baby from SIDS.
