  1. List of Legally Mandated Reportable Conditions and Timelines for Reporting
    The legal mandate to report is specified in Title 17 (Public Health) of the California Code of Regulations (CCR) §2500 for Reportable Diseases and Conditions. Failure to report is a misdemeanor and a citable offense. Immediate reporting of animal rabies and bite reporting is  mandated in CCR Title 17 §2606.

    For STD's or TB, use the forms under those categories below. In the absence of a condition-specific form, please use the General Confidential Morbidity Report form.
